If you are hired by TTA, we will have you complete the next steps.

Paperwork

 

  • W-9 and Safekids

All Paperwork is now Digital. Please follow the below instructions

  • Fill out 2 pages and create a digital signature by double clicking where it says “Click to Sign”
    ON BOTH PAGES 

    Make sure to fill out: Name, Address, Email, Phone, Bank Info and Social Security Number

  • Click: Save
  • Click: Email me the Link and you are DONE!!

Fingerprinting

  • Step 1) Click This Link-  coachlivescan -and Complete the section for APPLICATION INFORMATION in the LIVESCAN FORM.  Please fill out all information but Misc. NumberPrint 3 Copies and proceed to step 2
  • Step 2)  Find a location close to you and go get fingerprinted.  There is a small fee associated with getting printed that will be your responsibility.Remember to bring in 3 copies of the FORM

  • All Contractors must be CPR/AED Certified
